    • Attachments are allowed.
    • Multiple lead documents per filing are allowed.

    Initial Filings

    • Case Information Sheet should be submitted as the last page of a petition or as an attachment.
    • Exhibits should be merged to the lead document.
    • Service Request Form can be an attachment (not a lead document).
    • If desired, under Court Services order copies of the pleading for the clerk to attach to citation.
    • The addresses of the plaintiff are required. The addresses of the defendants are required.
    • In preparing an Original Petition all parties should be listed.
    • Although additional documents can be uploaded as attachments, you may upload any document you want file marked as a lead document in a new filing.

    Subsequent Filings

    • Items not needing a file stamp (proposed orders, notices, etc) should have a cover letter as the lead document.
    • Motions should be merged with the lead document.
    • Exhibits should be filed as an attachment to an appropriate lead document.
    • Proposed orders should be filed as an attachment with a cover letter as the lead document

    Request Citations, Etc.

    • Service Request Form is required when requesting service of citation on a subsequent filing.
    • If copies requested, request enough copies for each page of the pleading.
